What to bring with you.
Most of what we need will already be on the printed patient form. A few things help us move faster on the day of your visit.

- Any recent X-rays, MRI reports, or relevant medical records
- List of medications or supplements
- Comfortable clothing if possible
- Questions you want to ask the doctor

How long does the first visit take?
First visits are usually longer than regular visits because we take time to understand your history, symptoms, and goals. Plan for 45 to 60 minutes. If you are unsure how much time to allow, please call the clinic before your appointment.
Should I fill out the form before I arrive?
Yes. Download and print the new patient form, fill it out at home, and bring it with you. Completing it ahead saves time at the clinic and gives Dr. Katz a chance to review your details before you sit down together. No printer at home? Call the clinic and we will have a copy ready for you to fill out when you arrive.
Will I receive treatment on the first visit?
That depends on your situation. If care is appropriate, Dr. Katz will explain the recommended next step. In some cases, further evaluation may be needed first.
What should I wear?
Wear comfortable clothing that allows you to move easily. Athletic wear works well. We do not require gowns for most visits.
Do I need a referral?
In many cases, patients can book directly with the clinic. If your insurance or medical situation requires a referral, please check with your provider or contact the clinic.
What if I am nervous?
That is completely okay. We will walk you through the process, explain what we are doing, and answer your questions before moving forward. You are always welcome to ask anything.
Can I bring previous imaging or reports?
Yes. If you have recent X-rays, MRI reports, or other relevant records, please bring them with you. They can give us useful context for your visit.
